To ensure your documents reach their correct destination when processing LTD claims, please use the "Send LTD Document" button locate in your Secure Employer Account only for the specific records listed below:
- Employer and employee claim packet
- FMLA documents
- Time sheets or absence reports
- Attending physician statement (if you have received it)
- Any additional records provided by the member
Please do not use the "Send LTD Document" button in your employer secure account for the following documents, as they will be sent to Broadspire instead of ASRS:
- 65 Plus Member Waiver Form
- Health Insurance Premium Benefit Authorization Form
- Reimbursement of Medical and or Dental Cost Form
- Health Insurance Change/Delete Form
- ASRS Employer Health Annual Plan and Rate Survey
