When the necessary form or packet is required, please ensure that you fully complete and sign the form(s) before submitting it to the Arizona State Retirement System (ASRS).
Each form and packet serves a specific purpose, but let’s bring a few of them “In Focus.”
The 65+ Membership Waiver should be offered upon acceptance of employment to any employee whose employment start date is on or after their 65th birthday, and they are not already an active, inactive, retired, or disabled ASRS member. This form must be completed entirely, including the employee's signature and an authorized Payroll or Human Resource staff member's signature. In addition, it must be received by the ASRS within 30 days of the employee’s start date. We are also focusing on the Employer Vendor ACH Direct Deposit Form. This form is unique as it is the only form that’s optional and designed to bring convenience to our employer partners by authorizing the direct deposit of payments from ASRS to the employer.
This form also authorizes the ASRS and/or this bank to debit the account for the purpose of error corrections from the designated account.
