The ASRS has recently released a new form to help streamline the reporting of your Health Insurance Plans. The new Annual Employer Provided Health Insurance Plans and Premiums Report and the Employer Health Insurance Plan/Premium Breakdown Table forms are your one-stop shop to provide the ASRS with your updated annual Health Insurance Plans and Premiums for the following benefit year.
Please delete all the prior reporting forms you may have saved to your website or computer. Please discard all hard copies and download the new Annual Employer Provided Health Insurance Plans and Premiums Report and the Employer Health Insurance Plan/Premium Breakdown Table which is available in your secure employer account on the Forms & Packets page. The forms can be submitted to the ASRS via secure messaging and must be completed no later than 45 days prior to the month your new insurance begins.
